New Patient & Referrals

Rocky Mountain Neurology REQUIRES a referral to be seen even if your insurance does not require you to have a referral for a specialist. We will not schedule a new patient without a referral on file.

Referrals must be faxed to
866-383-0569 and include the following:
  • Diagnosis & reason for referral
  • Patient Demographics including contact information
  • Copay of ID & Insurance Cards front & back
  • Office visit notes, labs, and imaging
  • Testing orders for EEG, AEEG, Neurobehavioral testing, & EMG/NCS including extremities
PHREESIA

Rocky Mountain Neurology is now using PHREESIA electronic patient registration system to process and monitor our incoming referrals. Referrals take 24-48 hours to be processed and loaded into the PHREESIA system.

Due to this process, it can take up to 3 business days before patients are contacted to schedule.

Tricare referrals MUST include the following:
  • Diagnosis & reason for referral
  • Tricare approval (Addressed to Rocky Mountain Neurology)
  • Patient Demographics including contact information
  • Copy of ID & Insurance Cards front & back
  • Office visit notes, labs, and imaging
  • Testing orders for EEG, AEEG, Neurobehavioral testing, & EMG/NCS including extremities

**We will not schedule Tricare referrals until we have received ALL the items above**

Insurance Information

If your insurance plan requires a referral authorization from your primary care physician, YOU will need to contact your primary care physician or insurance company to be sure it has been obtained. If we have yet to receive authorization prior to your appointment time, we will reschedule.

Workers’ Compensation
  • We only accept workers’ compensation for EMG/NCS testing
  • You will need to have your workers’ compensation carrier/adjuster fax an approval letter to verify the following information to 303-790-2810
    • accident date
    • claim number
    • last office visit note
    • order for testing/procedures.
  • We will not contact you to schedule until we have received and spoken to your workers’ compensation carrier/adjuster

Rocky Mountain Neurology accepts most major insurance plans. However, with the frequent changes that happen in the insurance marketplace, it is a good idea for you to contact your insurance company prior to your appointment and verify if we are a participating provider with your plan.

 

If we are not in-network under your insurance plan and you do not have any out-of-network benefits, you may opt to self-pay for the appointment.
