Do you participate in my insurance plan?

Rocky Mountain Neurology accepts most major insurance plans. However, with the frequent changes that happen in the insurance marketplace, it is a good idea for you to contact your insurance company prior to your appointment and verify if we are a participating provider as per your plan.

Will a co-payment be collected for each visit?

Yes, a co-payment will be collected for each visit if you have one with your plan. We cannot waive or bill the co-payments that are required by your insurance as this is a violation of insurance rules. If you are unable to pay your co-payment at the time of your appointment, you will be rescheduled.

I received a large bill, can I set up a payment plan?

Yes, we allow payment plans to be set up for all patient balances. You may set up a payment plan in the office or by contacting our billing department.

Why must I show my insurance cards for each visit?

We are required to have a copy of your insurance and ID on-file for each year. We ask for the cards at every visit to ensure we have the most up-to-date information on file.

If my insurance refuses to pay for services received, will I be responsible for the charges?

Although we may estimate what your insurance company may pay, it is the insurance company that makes the final determination of your eligibility and benefits. If your insurance company is not contracted with us, you agree to pay any portion of the charges not covered by insurance, including but not limited to those charges above the usual and customary allowance. If we are out of network for your insurance company and your insurance pays you directly, you are responsible for payment and agree to forward the payment to us immediately.

What should I bring to my new patient appointment?

How do I request my medical records?
  • Patients
  • All non-patient medical records requests need to be faxed to 303-790-2810
Do you charge for medical records request, FMLA forms, or letter requests?
  • We do not charge patients or other providers for medical records requests
  • Legal, and all other non-patient or provider medical records requests will be invoiced based of the request
  • There is a form fee of $25 per form with a maximum of $75 when multiple forms submitted at one time
  • Letter request can range $25-$50 depending on the requirements per provider.
What is your appointment cancellation policy?
  • New Patient visits, follow-up visits, and virtual visits require 48 business hours’ notice to cancel to avoid charges
  • Appointments requiring the services of a translator require 72 business hours’ notice to cancel to avoid charges. This is because the translator services company we use requires 48 hours’ notice to cancel the request for a translator
  • Testing & procedure appointments require 72 business hours’ notice to cancel to avoid charges.

We send reminders and appointment confirmations starting 7 days prior to your scheduled appointment to allow adequate time for cancellations or reschedules.

When will I receive my test results?
  • Depending on what tests were ordered you provider or staff will call you to provide the results or you will receive a letter in the mail or through the patient portal.
  • If you have multiple tests ordered, your provider will wait to go over all the results at your next follow up appointment unless there is any time abnormal that needs to be address right away.
  • Some imaging results can take up to 4 weeks to get the results back
  • Neurobehavioral testing results take 6 weeks to receive, and results will only be given in person during the follow-up visit.
How can I contact my provider during & after business hours?

Rocky Mountain Neurology does not have any on-call or after-hours coverage. The best way to reach your provider is through your patient portal or to leave voicemail with the office

Do you accept pediatric patients?

Rocky Mountain Neurology does not see any patients under the age of 18

Do you accept Workers Compensation, Personal Injury, or Lein claims?
  • We accept Workers Compensation insurance for EMG testing appointments only.
  • Personal Injury or Lein claims can be seen but will be processed as self-pay and payment for services are required to be paid in full at the time of service. It will be the patient’s responsibility to seek reimbursement from the Lein or Personal Injury company. Liability cases will also be considered self-pay accounts. We do not accept attorney letters or contingency payments.
What is your Self-pay policy?
  • All self-pay patients are required to sign an agreement and keep a credit card on-file. All payments are due in full at the time of service and there are no payment plan options available.
  • If you have commercial insurance and are not eligible for Medicare, Tricare, or Medicaid and have a high deductible or are out of network with Rocky Mountain Neurology, you may elect to pay self-pay for services.
Do you have translator services available?
  • We offer translator services if requested by the patient
  • If you need a translator, you can request one when you schedule your appointment.
  • If you request a translator, you must provide 72 business hours’ notice to cancel or reschedule.
  • If no-show or cancel with less than 72 business hours’ notice a fee of $75 will be billed to cover the cost of the translator services.
How do I request a refill of my medication?
  • Patients should contact their pharmacy first when needing a medication refill. Pharmacies will either refill a patient’s medication on the spot or they will fax a generated refill request form to Rocky Mountain Neurology.
  • Rocky Mountain Neurology requires regularly scheduled office visits with their prescribing physician. The interval will vary based on the medication prescribed, if a patient has not been seen in over a year, refill requests will be denied by our physicians and an appointment will be required before consideration of a refill.
  • Prescription refills are authorized only during normal business hours Monday- Friday 8:00am – 4:00pm. Prescriptions will not be filled after hours or on the weekends or holidays.
