- Your Insurance Card; State identification or passport
- Physician referral forms if required by insurance
- A list of current prescriptions you are taking, including dose & frequency
- Pertinent information about your medical and surgical history
- Any pertinent test results/medical records/CDs you may have
We ask that you arrive for your first appointment 30 minutes early in order to to get checked in.
