- Your Insurance Card; State identification or passport
- Physician referral forms if required by insurance
- A list of current prescriptions you are taking, including dose & frequency
- Pertinent information about your medical and surgical history
- Any pertinent test results/medical records/CDs you may have
We ask that you arrive for your first appointment 15-20 minutes early in order to to get checked in.
During high volume times (due to construction), parking at the Aspen Building can be limited. Please be aware that each building provides complimentary valet service (tipping is not required, but highly recommended).
